We are hiring a Medical Social Worker (MSW) for a Hospice position providing care to patients in their homes in the Jefferson Hills, PA area.

What You’ll Do:

\n

\n Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life\n Conduct comprehensive psychosocial and bereavement assessments, identifying patient and family needs while developing care plans that support emotional, social, and end-of-life goals\n Provide counseling, emotional support, and guidance to patients and families, connecting them with resources and helping them navigate the end-of-life journey with clarity and dignity\n Apply critical thinking and compassionate problem-solving to address complex situations, advocate for patient wishes, and ensure accurate, timely, and compliant EMR documentation\n Work independently but never alone\n Travel to patient homes within assigned territory\n \n

MSW Qualifications

\n

\n Master’s in Social Work from graduate school accredited by the Council of Social Work\n One (1) to two (2) years post graduate social work experience in a health care setting\n Licensed MSW preferred\n Current driver’s license and ability to spend ~20% of your day driving to/from patient locations\n \n

\n
