We are seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our team at Patient Care Mental Health, Inc. Work Environment
This role is entirely remote, which provides flexibility and the ability to work from the comfort of your home while maintaining a healthy work-life balance. Our hospice center supports a multidisciplinary team environment that emphasizes collaboration among healthcare professionals, including physicians, nurses, and other social workers, to ensure integrated and effective care for our patients.
Key Responsibilities
- Conduct thorough bio-psychosocial evaluations to identify barriers to treatment and understand the psychosocial needs of patients and their families.
- Develop and implement individualized treatment plans based on evidence-based practices, utilizing both individual and group modalities.
- Work collaboratively with medical and support staff to formulate comprehensive care programs aimed at enhancing treatment compliance and coordination.
- Maintain accurate documentation of social work activities according to departmental standards, ensuring that all interactions and interventions are clearly recorded in patients' medical records.
- Provide education to patients and families about treatment processes, helping them to understand and accept the medical and therapeutic interventions recommended.
- Assist in assessing the advocacy needs of patients and coordinate with community resources to ensure appropriate support systems are in place.
- Remain current with available community resources to facilitate referrals and enhance service delivery for patients.
- Engage in ongoing professional development through training and education, focusing on enhancing clinical skills to maintain high standards of care.
- Contribute to departmental goals by completing case-related statistics and reports as required.
- Foster a therapeutic, safe, and compliant care environment by adhering to laws, regulations, and organizational policies.
Patient Population / Client Focus
This position centers on working with patients receiving hospice care, along with their families. The primary focus is on individuals facing significant health challenges, requiring emotional and psychological support throughout their transitions. With your skills and empathy, you will guide them towards understanding their condition, addressing their fears, and finding peace during difficult times.
Qualifications
- A Master's Degree in Social Work from an accredited graduate school.
- Current licensure as a Licensed Clinical Social Worker (LCSW) in the State of Florida is mandatory.
- Strong foundational knowledge of human development, aging processes, and cultural considerations in mental healthcare.
Preferred Experience
- At least 1 year of clinical experience, preferably within a health or mental health setting with a focus on terminally ill populations.
- Experience with case coordination and advocacy within community resources is advantageous.
Compensation and Benefits
The annual salary for this role ranges from USD 85,000 to 100,000, commensurate with experience and education. In addition to competitive pay, we offer a sign-on bonus and a comprehensive benefits package that includes health insurance, retirement planning options, and continuing education support.
