Work Environment
This is a fully remote position based in Colorado Springs, Colorado, allowing for a flexible work schedule that accommodates your personal commitments while fulfilling the needs of our patients. As a member of our multidisciplinary team, you will engage closely with other healthcare professionals, ensuring a cohesive approach to patient care from a distance.
Key Responsibilities
- Report directly to the Social Work Manager while evaluating, diagnosing, and treating a variety of psychosocial challenges faced by hospice patients and their families.
- Utilize clinical social work skills to assist patients in overcoming personal and environmental barriers that hinder their psychosocial functioning.
- Adhere to the NASW Code of Ethics and maintain professional standards of practice while delivering services.
- Conduct comprehensive bio-psychosocial evaluations to identify treatment barriers and enhance patients' health status, leading to improved resource utilization.
- Provide critical input on patients' psychosocial needs to medical and support staff, facilitating comprehensive care planning aimed at enhancing treatment compliance and coordination.
- Develop and implement individualized treatment plans incorporating both individual and group therapeutic modalities, regularly analyzing treatment outcomes to optimize care.
- Document social work activities and patient interactions in medical records in accordance with departmental standards.
- Educate patients and families on medical and therapeutic recommendations, fostering understanding and adherence to create pathways for optimal recovery.
- Advocate for patients and coordinate community resource accessibility to support their social needs, emphasizing client self-determination and empowerment.
- Complete case-related statistics and reporting in line with departmental requirements to contribute to overall service efficiency.
- Engage in ongoing professional development activities, including training relevant to specific clinical responsibilities.
- Comply with departmental policies and maintain a safe care environment while aligning with all relevant laws and regulations.
Patient Population / Client Focus
This role primarily focuses on patients receiving hospice care, who often face complex emotional and mental health challenges tied to end-of-life transitions. You will work with individuals and families from diverse backgrounds, ensuring that your approach is sensitive to their unique experiences and cultural contexts. This position allows you to make a profound difference in the lives of individuals facing serious illnesses.
Qualifications
- A Master's Degree in Social Work from an accredited graduate school is required, along with a current LCSW license from the State Board of Social Work Licensure.
- At least one year of experience in a clinical setting, ideally with a focus on mental health or healthcare, and experience with hospice or palliative care is preferred.
- Demonstrated proficiency in applying an understanding of aging processes, human development stages, and cultural patterns during the provision of care.
Preferred Experience
While a minimum of one year of clinical experience is required, candidates with additional experience in hospice or end-of-life care will be prioritized. Knowledge of community resources specific to mental health and patient advocacy is highly valued.
Compensation and Benefits
The annual salary for this full-time Licensed Clinical Social Worker position ranges from USD 85,000 to 100,000, commensurate with experience and qualifications. Additional benefits include health insurance options, a retirement savings plan, paid time off, and a SIGN-ON BONUS for new hires.
